y-Globulin Treatment of Acute Myocarditis in the Pediatric Population

چکیده
Background Myocardial damage in myocarditis is mediated, in part, by immunological mechanisms. High-dose intravenous y-globulin (IVIG) is an immunomodulatory agent that is beneficial in myocarditis secondary to Kawasaki disease, as well as in murine myocarditis. Since 1990, the routine management of presumed acute myocarditis at Children's Hospital, Boston, and Children's Hospital, Los Angeles, has included administration of high-dose IVIG. Methods and Results We treated 21 consecutive children presenting with presumed acute myocarditis with IVIG, 2 g/kg, over 24 hours, in addition to anticongestive therapies. A comparison group comprised 25 recent historical control patients meeting identical eligibility criteria but not receiving IVIG therapy. Left ventricular function was assessed during five time intervals: 0 to 7 days, 1 to 3 weeks, 3 weeks to 3 months, 3 to 6 months, and 6 to 12 months. At presentation, the IVIG and non-IVIG groups had comparable left ventricular enlargement and poor fractional shortening. Compared

Myocarditis and Meningitis during Early Sepsis in a Neonate with Streptococcus pseudopneumoniae: A Case Report
Although myocarditis is uncommon in neonates, a wide variety of infectious pathogens can result in myocarditis, including viruses, bacteria, rickettsia, fungi, and protozoa. Viruses are most often the infectious disease found to cause acute myocarditis. On the other hand, bacterial myocarditis (BM) is an unusual cause of infectious myocarditis. BM is commonly seen in the context of sepsis or as...
متن کاملA systematic review of intravenous gamma globulin for therapy of acute myocarditis
BACKGROUND Intravenous gamma globulin (IVGG) is commonly used in the management of acute myocarditis. The objective of this study was to systematically review the literature evaluating this practice. METHODS We conducted a comprehensive search (electronic databases, trials registries, conference proceedings, reference lists, contact with authors) to identify studies evaluating the use of IVGG...
متن کاملClinical outcome of acute myocarditis in children according to treatment modalities
PURPOSE There is currently little evidence to support intravenous immune globulin (IVIG) therapy for pediatric myocarditis. The purpose of our retrospective study was to assess the effects of IVIG therapy in patients with presumed myocarditis on survival and recovery of ventricular function and to determine the factors associated with its poor outcome. METHODS We reviewed all consecutive case...
متن کاملIn-hospital arrhythmia development and outcomes in pediatric patients with acute myocarditis.
Cardiac arrhythmias are a complication of myocarditis. There are no large studies of in-hospital arrhythmia development and outcomes in pediatric patients with acute myocarditis. This was a retrospective 2-center review of patients ≤21 years hospitalized with acute myocarditis from 1996 to 2012. Fulminant myocarditis was defined as the need for inotropic support within 24 hours of presentation....

BACKGROUND Kawasaki disease (KD) is an etiological illness that is relatively unknown and scarcely identified in Mexico; it affects children mainly aged 1-4 years, evolves with fever, vasculitis in diverse organs, and in the heart the disease mainly affects the coronary arteries. OBJECTIVES Our aim was to inform the clinical findings and evolution of 20 patients diagnosed with KD. MATERIALS...
